Job Description

Deliver high-quality, compassionate nursing care directly to patients in their homes as part of a dedicated healthcare team. This contract role is ideal for a detail-oriented, proactive Licensed Practical Nurse (LPN) with a strong background in home health and a commitment to supporting patient independence and well-being.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
  • Field-based role within an assigned territory
  • Workload includes managing 2532 points per week (approximately 35 patient visits per day)

Key Responsibilities:

  • Conduct skilled nursing visits, assess patient needs, and implement tailored care plans
  • Administer medications, perform wound care, and manage IV therapy as ordered
  • Monitor patient status and promptly communicate changes to the RN Case Manager or physician
  • Educate patients and caregivers on medication management, disease processes, and self-care techniques
  • Complete detailed and accurate documentation using the Home Care Home Base (HCHB) electronic charting system
  • Collaborate with interdisciplinary team members to coordinate care and maintain compliance with all care plans
  • Uphold infection control standards and ensure patient safety at all times
  • Participate in case conferences, team meetings, and ongoing professional training

Required Qualifications:

  • Active and unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or community-based care
  • Proficiency in Home Care Home Base (HCHB) documentation
  • Valid drivers license; reliable transportation with current insurance
  • Current BLS certification
  • Excellent clinical decision-making, time management, and communication skills
  • Ability to effectively manage a varied and independent caseload

Preferred Attributes:

  • Experience with Medicare-certified home health agencies
  • Knowledge of compliance and documentation protocols for home health
