Job Description
Provide skilled nursing care as a Home Health Licensed Practical Nurse (LPN) delivering patient-centered services in the comfort of patients' homes. This field-based role requires managing a caseload while ensuring compliance with home health standards and effective communication with patients, families, and healthcare teams.

Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans.
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy.
- Monitor and report changes in patient conditions to RN Case Managers or physicians.
- Educate patients and caregivers on disease management, medication, and self-care techniques.
- Document all care and communication accurately using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary teams to coordinate care and comply with care plans.
- Maintain infection control and ensure patient safety.
- Participate in case conferences, quality improvement, and training activities.
Qualifications:
- Active, unrestricted LPN license in the practicing state.
- Minimum of 1 year in home health, hospice, or community-based care.
- Proficiency with Home Care Home Base (HCHB) charting system.
- Valid drivers license and reliable vehicle with current insurance.
- Current BLS certification.
- Strong clinical judgment, communication, and time management skills.
- Ability to work independently and manage a dynamic caseload.
Preferred Experience:
- Medicare-certified home health agency background.
- Deep understanding of home health compliance and documentation.
- Excellent interpersonal skills with a patient-centered care approach.
Schedule:
- Full-time contract, Monday to Friday, 8:00 AM 5:00 PM (some flexibility for patient needs).
- Approximately 35 patient visits per day.
