Job Description
Provide compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) delivering services in patients homes within the Lewes, DE area. This role demands skilled clinical expertise, adherence to regulatory standards, and proficiency in documentation through Home Care Home Base (HCHB).
Schedule & Workload:
- Full-time contract position
- Monday Friday, 8:00 AM 5:00 PM (with occasional flexibility)
- Manage approximately 35 patient visits daily (2532 points per week)
- Field-based work requiring reliable personal transportation
Key Responsibilities:
- Perform skilled nursing assessments, develop individualized care plans, and implement nursing interventions
- Administer medications, provide wound care, and oversee IV therapy as ordered
- Monitor patient status changes and communicate with RN Case Managers or physicians promptly
- Educate patients and caregivers on disease management, medication use, and self-care
- Maintain accurate and compliant documentation in HCHB
- Collaborate effectively with the interdisciplinary healthcare team to coordinate care
- Uphold infection control protocols and patient safety standards
- Engage in case conferences, quality improvement initiatives, and ongoing training
Required Qualifications:
- Active, unrestricted LPN license for the state of practice
- Minimum one year of experience in home health, hospice, or similar community care
- Proficiency with Home Care Home Base (HCHB) electronic charting
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a diverse caseload
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Solid knowledge of home health compliance and documentation standards
- Empathetic and patient-centered communication skills
