Deliver essential nursing care and support to clients in their homes as a Licensed Practical Nurse (LPN) specializing in home health. This contract opportunity focuses on providing high-quality, patient-centered care, supporting patients independence, and ensuring their comfort and wellbeing through thoughtful nursing practice.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility as needed for patient care
• Full-time contract

Key Responsibilities:

• Conduct in-home skilled nursing visits, performing assessments and developing individualized care plans
• Administer medications, provide wound care, and manage IV therapies or other physician-ordered interventions
• Monitor and promptly report changes in patient health status to the RN Case Manager or physician
• Educate patients and caregivers on medication, disease management, and self-care practices
• Accurately document visits and communications using Home Care Home Base (HCHB)
• Collaborate effectively with the interdisciplinary care team to ensure a coordinated care approach
• Maintain infection control and safety protocols at all times
• Take part in quality improvement initiatives, training sessions, and case conferences

Qualifications:

• Current, unrestricted LPN license in the state
• Minimum of one year of relevant nursing experience in home health, hospice, or community-based care
• Proficient with Home Care Home Base (HCHB) charting system
• Valid drivers license, reliable vehicle, and current auto insurance
• Active BLS certification (Basic Life Support)
• Excellent clinical judgment, organizational, and communication skills
• Ability to work independently and manage a flexible, field-based caseload

Preferred:

• Experience in Medicare-certified home health settings
• Deep understanding of home health regulatory documentation
