Deliver patient-centered nursing care as a Licensed Practical Nurse (LPN) in a home health setting, supporting individuals in their homes within an assigned territory near Lees Summit, MO. This full-time, contract role offers a structured Monday through Friday schedule (8:00 AM 5:00 PM) with some flexibility to meet patient needs. Manage 2532 points weekly, equivalent to approximately 35 patient visits per day, using Home Care Home Base (HCHB) for documentation.
Responsibilities include conducting skilled nursing visits, administering medications, performing wound care, managing IV therapies, and monitoring patient conditions. You will educate patients and caregivers on self-care, collaborate with healthcare teams to coordinate care plans, maintain safety and infection control standards, and participate in ongoing training and quality improvement activities.
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of home health, hospice, or community-based care experience
- Proficient in Home Care Home Base (HCHB)
- Current BLS certification
- Valid drivers license with reliable insured vehicle
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based role
Preferred:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-centered approach
