Utilize your clinical expertise and communication skills to support patients and families, working closely with an interdisciplinary team to ensure optimal health outcomes.
What Youll Do:
- Deliver skilled nursing care in patients homes based on individualized care plans
- Monitor, assess, and document patient progress, making timely updates to care plans
- Administer medications and treatments according to state guidelines
- Educate patients and families on illness management, preventive care, and safe practices
- Maintain thorough and accurate records within HomeCare HomeBase (HCHB) EMR
- Partner with registered nurses, therapists, and other care providers for comprehensive care coordination
Required Qualifications:
- Active LPN license
- At least one year of recent experience in home health
- Proficiency using HomeCare HomeBase (HCHB) EMR
- Well-developed clinical assessment and documentation abilities
- Reliable transportation for patient visits
- Strong organizational skills and the ability to manage an independent caseload
