Schedule/Hours:

  • Current, active LPN license.
  • At least one year of recent experience in home health nursing.
  • Demonstrated proficiency with HomeCare HomeBase (HCHB) electronic medical records.
  • Strong clinical assessment and meticulous documentation abilities.
  • Reliable transportation for travel to patient residences.
  • Self-motivated approach to managing a dynamic caseload.

Primary Duties and Responsibilities:

  • Deliver skilled nursing care, following individualized plans tailored to patient needs.
  • Monitor and accurately record patient progress and update care plans as necessary.
  • Administer medications and perform prescribed treatments per state regulations.
  • Provide education to patients and family members on disease management and preventive care strategies.
  • Utilize HomeCare HomeBase (HCHB) EMR for complete, up-to-date recordkeeping.
  • Collaborate effectively with registered nurses, therapists, and healthcare providers to deliver coordinated, high-quality care.
