As a Home Health Licensed Practical Nurse (LPN), you will play a central role in supporting patient independence, recovery, and overall well-being. This contract opportunity offers a dynamic, field-based position working closely with a collaborative interdisciplinary team, focused on patient-centered care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility for patient needs
Key Responsibilities:
- Conduct skilled nursing assessments and implement individualized care plans for home health patients
- Administer medications, provide wound care, and manage IV therapy or other clinical interventions
- Monitor patient conditions closely and communicate changes to the RN Case Manager or physician
- Deliver patient and caregiver education on self-care, disease management, and medication administration
- Document all care accurately in Home Care Home Base (HCHB)
- Collaborate with healthcare professionals to coordinate comprehensive patient care
- Adhere to infection control standards and prioritize safety at every visit
- Participate in case conferences, quality improvement initiatives, and ongoing professional development
Required Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or community-based care
- Skilled in using Home Care Home Base (HCHB)
- Valid drivers license and reliable transportation with current insurance
- Current BLS (Basic Life Support) certification
- Excellent time management, clinical, and communication skills
- Ability to manage a caseload independently in a field setting
Preferred Experience:
- Background with Medicare-certified home health agencies
- Strong knowledge of home health compliance and documentation
