Job Description
Delivering essential in-home care, this position is ideal for an experienced and compassionate Licensed Practical Nurse (LPN) passionate about supporting patients independence and wellness. Serving clients in the community, this full-time field-based contract involves providing top-quality nursing care directly in patients homes, with an emphasis on education, recovery, and collaborative care planning.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility based on patient needs)
- Field-based within assigned home health territory
- Expected workload of 2532 points per week (generally 35 visits daily)
Key Responsibilities:
- Complete skilled nursing visits and develop individualized patient care plans
- Administer medications, perform wound care, and deliver IV therapy or other interventions per medical orders
- Monitor patient conditions and promptly report changes to the RN Case Manager or physician
- Provide patient and caregiver education on self-care, medication management, and disease processes
- Accurately document all care and communication using the Home Care Home Base (HCHB) platform
- Uphold infection control principles and ensure patient safety
- Participate in interdisciplinary team meetings, case conferences, and quality initiatives
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or community-based LPN experience
- Proficiency using Home Care Home Base (HCHB) required
- Current BLS certification
- Valid drivers license and reliable, insured vehicle
- Strong clinical judgment, communication, and time management skills
- Ability to thrive independently while managing a dynamic field caseload
Preferred:
- Medicare-certified home health agency experience
- Expertise in home health compliance and documentation
