Job Description
Deliver compassionate, patient-centered nursing care as a Home Health Licensed Practical Nurse (LPN) supporting individuals in their own homes. This full-time contract position is ideal for an experienced LPN skilled in providing skilled nursing visits while promoting patient independence, education, and recovery.
Responsibilities include conducting thorough assessments, administering medications and clinical interventions such as wound care and IV therapy, and monitoring patient conditions. You will collaborate closely with interdisciplinary teams, maintain accurate documentation using Home Care Home Base (HCHB), and ensure compliance with safety and infection control standards. Participation in case conferences and quality improvement initiatives is also required.
Key Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable insured vehicle (mileage reimbursement provided)
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based environment managing a dynamic caseload
Preferred Experience:
- Familiarity with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Excellent interpersonal skills with a focus on patient-centered care
Schedule and Workload:
- Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility
- Approximately 2532 points per week (35 patient visits daily)
- Field-based visits within an assigned territory
