Make a significant impact as a Home Health Licensed Practical Nurse (LPN), supporting patients in their homes by delivering high-quality, patient-centered care. Youll play a critical role in fostering independence and recovery while providing ongoing education and compassionate support to patients and their families. This field-based position offers a consistent weekday schedule with occasional flexibility to accommodate patient needs.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, contract
- Manage 2532 points per week (roughly 35 home visits per day)
Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum one year of recent experience in home health, hospice, or similar community-based care setting
- Proficiency with Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current vehicle insurance
- Current Basic Life Support (BLS) certification
- Strong time management, clinical judgment, and communication skills
- Ability to work independently and efficiently in a field-based environment
Preferred Experience:
- Familiarity with Medicare-certified home health agencies
- Knowledge of home health compliance and documentation standards
- Excellent interpersonal skills and a patient-focused approach
Primary Responsibilities:
- Provide skilled nursing visits and implement individualized care plans
- Administer medications, deliver wound care, and manage other clinical interventions as prescribed
- Monitor and report any changes in patient condition to the RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care
- Ensure meticulous documentation using HCHB and maintain compliance with all regulations
- Collaborate with the interdisciplinary healthcare team to coordinate patient care
