Job Description

Deliver meaningful care and support to patients in their homes as a Licensed Practical Nurse with home health expertise. This contract-based opportunity suits professionals committed to clinical excellence and patient advocacy in a field-based setting. Youll work collaboratively with an interdisciplinary team, manage a dynamic caseload, and directly impact patients quality of life.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required to address patient needs

Key Responsibilities:

  • Conduct skilled nursing visits, assessing, planning, and implementing individualized home health care
  • Administer medications, perform wound care, and manage IV therapy or other doctor-ordered interventions
  • Monitor patient conditions and promptly report findings to RN Case Managers or physicians
  • Educate patients and caregivers on disease management, medication use, and self-care strategies
  • Accurately document assessments and care provided using Home Care Home Base (HCHB)
  • Collaborate with an interdisciplinary team to ensure goal-focused, compliant care coordination
  • Adhere strictly to infection control policies and prioritize patient safety
  • Participate in case conferences, quality improvement initiatives, and ongoing training

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least 1 year of experience in home health, hospice, or community-based care
  • Proficiency with Home Care Home Base (HCHB) charting
  • Valid drivers license, reliable vehicle, and current insurance
  • BLS (Basic Life Support) certification
  • Excellent clinical judgment, time management, and communication skills
  • Ability to work independently and manage a variable caseload

Preferred:

  • Experience with Medicare-certified home health agencies and home health documentation compliance
  • Strong interpersonal abilities and a compassionate, patient-centered care approach
