Advance your nursing career by providing compassionate, patient-focused care as a Home Health Licensed Practical Nurse (LPN). This contract opportunity requires delivering skilled clinical services in patient homes, supporting recovery and independence throughout the assigned territory.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Occasional flexibility required according to patient needs

Key Responsibilities:

• Conduct skilled nursing visits to assess, implement, and update individualized care plans for home health patients
• Administer medications, manage wound care, and perform IV therapy or other clinical procedures per provider orders
• Monitor and document changes in patient conditions, promptly communicating updates to the RN Case Manager or physician
• Educate patients and caregivers on disease management, medication administration, and self-care best practices
• Ensure thorough, accurate documentation using Home Care Home Base (HCHB) software
• Collaborate with interdisciplinary healthcare teams to ensure compliance and optimal patient outcomes
• Maintain strict infection control and safety standards at all times
• Participate in case conferences, ongoing quality improvement, and required continuing education

Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum one year of recent experience in home health, hospice, or community-based care
• Proven proficiency with Home Care Home Base (HCHB)
• Valid drivers license, reliable vehicle, and current auto insurance
• Current BLS certification
• Excellent time management, communication, and clinical judgment
• Ability to work independently and manage a varying caseload

Preferred Experience:

• Background with Medicare-certified home health agencies
• In-depth knowledge of home health compliance and documentation requirements
• Strong interpersonal skills and patient-centered approach
