Schedule/Hours:

Key Qualifications:
- Active LPN license in good standing
- At least one year of recent home health nursing experience
- Proficient in HomeCare HomeBase (HCHB) EMR documentation
- Exceptional clinical assessment and organizational skills
- Able to manage caseload and prioritize tasks effectively
- Reliable transportation to perform home visits
Key Responsibilities:
- Deliver skilled nursing care to patients in private residences following individualized care plans
- Monitor, assess, and document patient progress, making updates to care plans as necessary
- Administer medications and perform treatments in accordance with state nursing guidelines
- Provide patient and family education on ongoing care, management techniques, and health maintenance
- Maintain accurate and timely records within the HomeCare HomeBase (HCHB) system
- Collaborate with registered nurses, therapists, and other healthcare partners to ensure high-quality, comprehensive patient care
