A Licensed Practical Nurse (LPN) specializing in home health care is needed to provide compassionate, patient-focused nursing visits in a field-based setting. This full-time contract opportunity offers the chance to deliver skilled care to patients in the comfort of their own homes throughout an assigned territory in La Grange, KY.

Key Responsibilities:

• Conduct skilled nursing assessments and implement individualized care plans.
• Administer medications, perform wound care, and manage clinical interventions such as IV therapy.
• Monitor patient conditions and communicate changes promptly to supervising clinicians.
• Educate patients and caregivers on disease management, medications, and self-care techniques.
• Accurately document all care using Home Care Home Base (HCHB) software.
• Collaborate with interdisciplinary teams to ensure compliance with care plans.
• Maintain infection control and patient safety standards.
• Participate in case conferences and quality improvement initiatives.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum of 1 year of experience in home health, hospice, or similar community-based care.
• Proficient with Home Care Home Base (HCHB) electronic charting.
• Valid drivers license, reliable vehicle, and current insurance.
• Current Basic Life Support (BLS) certification.
• Strong clinical judgment, communication, and time management skills.
• Ability to work independently and manage a dynamic caseload.

Preferred:

• Experience with Medicare-certified home health agencies.
• Familiarity with home health compliance and documentation requirements.
• Excellent interpersonal skills with a patient-centered approach.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility to meet patient needs.
• Approximately 2532 points weekly (35 patient visits per day).
