Job Description
Deliver high-quality care and meaningful support as a Home Health Licensed Practical Nurse (LPN). This contract position is perfect for a driven nursing professional skilled in field-based, patient-centered care who enjoys promoting recovery and independence for clients in their own homes.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Key Responsibilities:
- Complete skilled nursing visits, assessing and providing care based on individualized patient needs
- Administer medications, manage wound care, perform IV therapy, and provide other ordered clinical interventions
- Closely monitor patient conditions, promptly communicating changes to care managers or physicians
- Deliver education to patients and caregivers on disease processes, self-care techniques, and proper medication management
- Thoroughly document all care, assessments, and communications using Home Care Home Base (HCHB)
- Coordinate with interdisciplinary healthcare team members to deliver cohesive patient care and maintain compliance with established plans
- Follow infection control protocols and uphold the highest standards of patient safety at every visit
- Join case conferences, contribute to quality improvement, and participate in ongoing education
Qualifications:
- Current, unrestricted LPN license in the state of practice
- Minimum one year of LPN experience in a home health, hospice, or related community-based role
- Proficient in Home Care Home Base (HCHB)
- Valid drivers license, reliable transportation, and current auto insurance
- Active BLS certification
- Strong time management, clinical judgment, and independent decision-making skills
Preferred Skills:
- Medicare-certified home health experience
- Advanced understanding of home health documentation standards and compliance
- Exceptional communication and interpersonal skills
