Job Description

Schedule / Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required to meet patient needs

Key Qualifications:

  • Active, unrestricted LPN license for this state
  • At least 1 year of experience in home health, hospice, or a community-based care setting
  • Strong proficiency with Home Care Home Base (HCHB) charting
  • Current BLS (Basic Life Support) certification
  • Valid drivers license, reliable vehicle, and current auto insurance
  • Excellent clinical judgment, time management, and communication skills
  • Demonstrated ability to work independently in a field-based environment

Preferred Experience:

  • Familiarity with Medicare-certified home health agencies
  • In-depth knowledge of home health compliance and documentation protocols
  • Natural ability to deliver compassionate, patient-centered care

Primary Responsibilities:

  • Perform skilled nursing visits, assess patient needs, and implement individualized care plans
  • Provide clinical interventions including wound care, medication administration, and IV therapy
  • Monitor patient progress and communicate changes to the healthcare team
  • Educate patients and caregivers on medications, disease management, and self-care
  • Maintain thorough documentation in HCHB, ensuring accuracy and regulatory compliance
  • Participate in case conferences, quality improvement activities, and ongoing training
  • Uphold infection control standards, prioritizing patient safety
