Home Health Licensed Practical Nurse (LPN) Kennesaw, GA (Contract)

An opportunity for a dedicated Home Health Licensed Practical Nurse (LPN) to deliver compassionate, patient-focused nursing care within patients homes. Position Details:

  • Contract Type: Full-time, contract
  • Schedule: Monday Friday, 8:00 AM 5:00 PM (with occasional scheduling flexibility)
  • Workload: Approximately 2532 patient care points weekly (35 home visits daily)
  • Setting: Patient homes within an assigned territory
  • Charting System: Home Care Home Base (HCHB) proficiency required
  • Travel: Reliable transportation required; mileage reimbursement provided

Responsibilities:

  • Conduct skilled nursing visits assessing, planning, and implementing individualized home care plans
  • Administer medications, wound care, and manage IV therapy or other prescribed clinical interventions
  • Monitor patient conditions, communicate changes promptly to RN Case Manager or physicians
  • Educate patients and caregivers on disease management, medication use, and self-care techniques
  • Document assessments and care accurately using HCHB for compliance with regulatory standards
  • Collaborate closely with the interdisciplinary healthcare team to coordinate comprehensive care
  • Maintain infection control and ensure patient safety in all home settings
  • Participate in case conferences, quality improvement initiatives, and required professional training

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of experience in home health, hospice, or similar community-based care
  • Proficiency with Home Care Home Base (HCHB) documentation system
  • Valid drivers license and reliable vehicle with current insurance
  • Current BLS (Basic Life Support) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently managing a dynamic caseload

Preferred:

  • Experience with Medicare-certified home health agencies
  • Solid understanding of home health compliance and documentation standards
  • Excellent interpersonal skills with a patient-centered care philosophy
