Home Health Licensed Practical Nurse (LPN) Kennesaw, GA (Contract)
An opportunity for a dedicated Home Health Licensed Practical Nurse (LPN) to deliver compassionate, patient-focused nursing care within patients homes. This role demands clinical expertise and strong communication to promote patient independence, education, and recovery while ensuring compliance with home health standards.

Position Details:
- Contract Type: Full-time, contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM (with occasional scheduling flexibility)
- Workload: Approximately 2532 patient care points weekly (35 home visits daily)
- Setting: Patient homes within an assigned territory
- Charting System: Home Care Home Base (HCHB) proficiency required
- Travel: Reliable transportation required; mileage reimbursement provided
Responsibilities:
- Conduct skilled nursing visits assessing, planning, and implementing individualized home care plans
- Administer medications, wound care, and manage IV therapy or other prescribed clinical interventions
- Monitor patient conditions, communicate changes promptly to RN Case Manager or physicians
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Document assessments and care accurately using HCHB for compliance with regulatory standards
- Collaborate closely with the interdisciplinary healthcare team to coordinate comprehensive care
- Maintain infection control and ensure patient safety in all home settings
- Participate in case conferences, quality improvement initiatives, and required professional training
Qualifications:
- Active, unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or similar community-based care
- Proficiency with Home Care Home Base (HCHB) documentation system
- Valid drivers license and reliable vehicle with current insurance
- Current BLS (Basic Life Support) certification
- Strong clinical judgment, time management, and communication skills
- Ability to work independently managing a dynamic caseload
Preferred:
- Experience with Medicare-certified home health agencies
- Solid understanding of home health compliance and documentation standards
- Excellent interpersonal skills with a patient-centered care philosophy
