Job Description
Advance your nursing career as a dedicated Home Health Licensed Practical Nurse (LPN), providing direct, compassionate care to patients in their homes. This full-time contract opportunity is ideal for professionals who excel in independent field work and are committed to delivering patient-centered care within the home health setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility as needed for patient care
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and deliver individualized home health care
- Administer medications, wound care, and clinical interventions, including IV therapy as ordered
- Monitor and report any changes in patient condition to RN Case Manager or physician
- Educate patients and their families on disease management and self-care strategies
- Accurately document all patient assessments, interventions, and communications using Home Care Home Base (HCHB)
- Collaborate with the healthcare team to ensure effective care coordination and compliance with care plans
- Uphold infection control and patient safety protocols at all times
- Participate in case conferences, continuous improvement initiatives, and required training
Required Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Valid drivers license, reliable vehicle, and current auto insurance
- Current Basic Life Support (BLS) certification
- Strong clinical, organizational, and communication abilities
Preferred Experience:
- Work history with Medicare-certified agencies
- Familiarity with home health regulations and compliance standards
- Patient-focused approach and excellent interpersonal skills
