Job Description

Delivering skilled nursing care to patients in a home health setting, this Licensed Practical Nurse role offers an opportunity to make a difference in the lives of geriatric clients. Utilizing your expertise and compassion, you will provide high-quality care and education, while working autonomously and efficiently in the community. This contract assignment requires a strong background in home health and proficiency with HomeCare HomeBase (HCHB) EMR.

Schedule/Hours:

  • Contract position; scheduling details discussed during interview

Key Qualifications:

  • Current, active LPN license
  • At least one year of recent experience in home health, specifically with geriatric patients
  • Proficiency with HomeCare HomeBase (HCHB) EMR system
  • Reliable transportation, and ability to travel to patient homes
  • Excellent clinical assessment skills and thorough documentation abilities
  • Capacity to work both independently and as part of an interdisciplinary team

Primary Responsibilities:

  • Deliver skilled nursing care and interventions based on established care plans
  • Monitor patient progress, updating plans and communicating changes as necessary
  • Administer medications and perform treatments under state guidelines
  • Educate patients and their families on disease management and prevention techniques
  • Consistently maintain accurate and up-to-date records using HCHB EMR
  • Collaborate with RNs, therapists, and other healthcare professionals for coordinated, comprehensive care
