Schedule/Hours:
- Contract assignment with potential for extensions
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes, following established care plans
- Monitor patient progress and update care plans as necessary
- Administer medications and perform treatments according to state regulations
- Educate patients and families on disease management and preventive care
- Accurately maintain clinical records using HomeCare HomeBase (HCHB) EMR
- Collaborate with registered nurses, therapists, and other healthcare providers for comprehensive, coordinated care
Qualifications:
- Active LPN license
- At least one year of recent experience in home health
- Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment skills and documentation abilities
- Reliable transportation to visit patients at home
- Strong organizational skills and the capability to work autonomously
