Schedule/Hours:

  • Contract assignment with potential for extensions

Key Responsibilities:

  • Deliver skilled nursing care to patients in their homes, following established care plans
  • Monitor patient progress and update care plans as necessary
  • Administer medications and perform treatments according to state regulations
  • Educate patients and families on disease management and preventive care
  • Accurately maintain clinical records using HomeCare HomeBase (HCHB) EMR
  • Collaborate with registered nurses, therapists, and other healthcare providers for comprehensive, coordinated care

Qualifications:

  • Active LPN license
  • At least one year of recent experience in home health
  • Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR
  • Excellent clinical assessment skills and documentation abilities
  • Reliable transportation to visit patients at home
  • Strong organizational skills and the capability to work autonomously
