A contract opportunity is open for an experienced Licensed Practical Nurse (LPN) with a background in home health and a commitment to providing exceptional support in a dynamic, patient-focused environment.
Schedule/Hours:
- Flexible contract hours, with a focus on patient needs in a home health setting.
Qualifications:
- Active LPN license
- At least one year of recent experience in home health
- Proficiency with HomeCare HomeBase (HCHB) EMR required
- Strong clinical assessment and documentation skills
- Reliable transportation to conduct home visits
- Ability to work independently and effectively manage a caseload
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes according to individualized care plans
- Monitor patient progress and update care plans as appropriate
- Administer medications and perform treatments following state and regulatory guidelines
- Educate patients and families on ongoing care management and disease prevention
- Document all interventions and communication in HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare team members to ensure comprehensive care
