A highly skilled and motivated Licensed Practical Nurse (LPN) is needed to deliver exceptional home health care in a field-based setting. This contract opportunity focuses on providing quality, patient-centered services to individuals in their homes, promoting independence, education, and optimal recovery.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

• Conduct skilled nursing visits, assessing, planning, and implementing individualized care plans for home health patients.
• Administer medications, perform wound care, and manage IV therapy or additional clinical interventions as ordered.
• Monitor and document changes in patient conditions, reporting promptly to the RN Case Manager or physician when needed.
• Educate patients and caregivers on disease processes, medication administration, and self-care techniques.
• Ensure accurate and timely documentation of all care and communication, utilizing Home Care Home Base (HCHB).
• Collaborate closely with the interdisciplinary healthcare team to coordinate and adjust patient care as needed.
• Uphold infection control practices and maintain a safe environment for all patients.
• Attend case conferences, participate in quality improvement activities, and complete required training.

Qualifications:

• Active, unrestricted LPN license in the state of practice.
• Minimum of 1 year of experience in home health, hospice, or similar community-based care setting.
• Proficiency with Home Care Home Base (HCHB) is required.
• Valid drivers license, reliable transportation, and current auto insurance.
• Current BLS (Basic Life Support) certification.
• Strong clinical judgment, excellent time management, and effective communication skills.
• Ability to work independently with a dynamic patient caseload.

Preferred Experience:

• Knowledge of Medicare-certified home health agency standards.
• Solid grasp of home health compliance and documentation.
• Excellent interpersonal skills and a patient-oriented approach.
