Job Description
A highly skilled and motivated Licensed Practical Nurse (LPN) is needed to deliver exceptional home health care in a field-based setting. This contract opportunity focuses on providing quality, patient-centered services to individuals in their homes, promoting independence, education, and optimal recovery.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
Key Responsibilities:
- Conduct skilled nursing visits, assessing, planning, and implementing individualized care plans for home health patients.
- Administer medications, perform wound care, and manage IV therapy or additional clinical interventions as ordered.
- Monitor and document changes in patient conditions, reporting promptly to the RN Case Manager or physician when needed.
- Educate patients and caregivers on disease processes, medication administration, and self-care techniques.
- Ensure accurate and timely documentation of all care and communication, utilizing Home Care Home Base (HCHB).
- Collaborate closely with the interdisciplinary healthcare team to coordinate and adjust patient care as needed.
- Uphold infection control practices and maintain a safe environment for all patients.
- Attend case conferences, participate in quality improvement activities, and complete required training.
Qualifications:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 year of experience in home health, hospice, or similar community-based care setting.
- Proficiency with Home Care Home Base (HCHB) is required.
- Valid drivers license, reliable transportation, and current auto insurance.
- Current BLS (Basic Life Support) certification.
- Strong clinical judgment, excellent time management, and effective communication skills.
- Ability to work independently with a dynamic patient caseload.
Preferred Experience:
- Knowledge of Medicare-certified home health agency standards.
- Solid grasp of home health compliance and documentation.
- Excellent interpersonal skills and a patient-oriented approach.
