Job Description

Deliver compassionate, patient-focused nursing care as a Home Health Licensed Practical Nurse (LPN) serving individuals in their homes. This full-time contract opportunity offers a field-based setting with a manageable caseload, allowing you to make a meaningful impact on patients independence and recovery.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM (some flexibility as patient needs arise)
  • Approximately 35 patient visits per day (2532 points weekly)

Key Responsibilities:

  • Conduct skilled nursing visits to assess, plan, and deliver individualized care in patients homes
  • Administer medications, perform wound care, manage IV therapy, and other clinical interventions
  • Monitor patient condition changes and communicate promptly with RN Case Manager or physicians
  • Educate patients and caregivers on disease management and self-care techniques
  • Document all clinical assessments and interventions accurately using Home Care Home Base (HCHB)
  • Collaborate with healthcare team members to coordinate care and ensure compliance with care plans
  • Uphold infection control standards and patient safety consistently
  • Participate in case conferences, quality improvement initiatives, and mandatory training

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of home health, hospice, or community-based care experience
  • Proficiency with Home Care Home Base (HCHB) charting system
  • Valid drivers license and reliable vehicle with current insurance
  • Current BLS certification
  • Strong clinical judgment, communication skills, and ability to work independently

Preferred:

  • Experience with Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation standards
  • Patient-centered mindset and excellent interpersonal skills
