A rewarding opportunity awaits Licensed Practical Nurses passionate about delivering comprehensive care in a home health setting. As a field-based clinician, youll provide hands-on nursing interventions, patient education, and compassionate support while helping individuals achieve maximum independence in the comfort of their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
• Full-time, contract assignment
• Manage 2532 points per week (approximately 35 patient visits per day)

Key Qualifications:

• Active, unrestricted LPN license in the state of practice
• Minimum one year of experience in home health, hospice, or a similar community-based care role
• Proficiency with Home Care Home Base (HCHB) system
• Valid drivers license, reliable transportation, and current insurance
• Current BLS certification
• Strong clinical judgment, time management, and communication abilities
• Capability to work autonomously in a field environment and handle a dynamic caseload

Preferred Experience:

• Previous work with Medicare-certified home health agencies
• Detailed knowledge of home health compliance standards and care documentation
• Commitment to patient-centered care and excellent interpersonal skills

Primary Responsibilities:

• Provide skilled nursing visits and individualized care for home-based patients
• Administer medications, perform wound care, manage IV therapies, and execute clinical orders
• Monitor and report patient condition changes to Case Managers or physicians
• Educate patients and caregivers on self-care, medications, and disease processes
• Ensure accurate, timely documentation via Home Care Home Base (HCHB)
• Collaborate with the care team to coordinate services and uphold quality standards
• Maintain infection control and promote patient safety at all times
• Engage in case conferences, complete training, and participate in quality improvement

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
