Deliver vital care and support to geriatric patients in a home health environment as a Licensed Practical Nurse (LPN). Schedule/Hours:
- Contract position
- Home health visits required
Qualifications:
- Active, unencumbered LPN license
- At least one year of recent home health nursing experience
- Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR system
- Strong clinical skills in assessment, care planning, and documentation
- Reliable personal transportation to conduct home visits
- Excellent communication skills and the ability to educate patients and families
Responsibilities:
- Deliver skilled nursing care in patients homes, following individualized care plans
- Monitor and document patient status; update care plans as patient needs change
- Administer medications and perform prescribed treatments in accordance with state guidelines
- Educate patients and families about condition management and preventive care
- Maintain meticulous records in HomeCare HomeBase (HCHB) EMR
- Collaborate with RNs, therapists, and other healthcare professionals to ensure high-quality, coordinated care
