Schedule/Hours:

Essential Qualifications:
- Active and unencumbered LPN license
- At least one year of recent home health nursing experience
- Proficiency in HomeCare HomeBase (HCHB) electronic medical records system
- Strong clinical assessment and documentation abilities
- Reliable transportation for in-home patient visits
- Ability to manage caseloads independently and efficiently
Primary Responsibilities:
- Deliver skilled nursing care tailored to patient care plans in home settings
- Administer medications and carry out treatments as per state regulations
- Monitor patient progress, documenting updates, and revising care plans as necessary
- Educate patients and their families on disease management and preventive health measures
- Maintain meticulous records through HomeCare HomeBase (HCHB)
- Collaborate effectively with registered nurses, therapists, and other healthcare professionals to ensure coordinated, comprehensive care
