Job Description
Make a meaningful impact as a Home Health Licensed Practical Nurse (LPN) providing quality, compassionate care to patients in a field-based setting. This contract position is centered on empowering individuals to recover and manage their conditions at home, ensuring safety and compliance every step of the way. Youll work closely with patients, families, and a multidisciplinary team, offering education, skilled nursing care, and consistent communication.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Primary responsibilities:
- Complete skilled nursing visits to assess and implement personalized care plans
- Provide medication administration, wound care, IV therapy, and other clinical services as needed
- Monitor, document, and promptly communicate changes in patient conditions to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medications, and home-care techniques
- Accurately chart all services and assessments using Home Care Home Base (HCHB)
- Collaborate with a multidisciplinary team to coordinate care and uphold best practices
- Uphold infection control protocols and ensure patient safety at all times
- Participate in case conferences, staff meetings, and quality improvement activities
Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of home health, hospice, or similar community-based experience
- Proficiency in Home Care Home Base (HCHB) required
- Current BLS (Basic Life Support) certification
- Reliable vehicle, valid drivers license, and current insurance
- Strong clinical skills, communication, and organizational abilities
- Preferred: Experience with Medicare-certified home health agencies and best practices in home health compliance
