Job Description
Deliver compassionate, patient-focused nursing care as a Licensed Practical Nurse (LPN) in a home health setting within Imperial, MO. This full-time contract position involves providing skilled nursing services directly in patients homes, promoting independence, education, and recovery, while maintaining strong communication with patients, families, and the interdisciplinary healthcare team.
Responsibilities:
- Conduct nursing visits to assess, plan, and implement personalized care for home health patients
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy
- Monitor patient condition changes and promptly report concerns to the RN Case Manager or physician
- Educate patients and caregivers on disease management, medication use, and self-care techniques
- Accurately document care and communications using Home Care Home Base (HCHB) system
- Collaborate with healthcare team members to coordinate care and ensure compliance with care plans
- Maintain infection control standards and prioritize patient safety
- Engage in case conferences, quality improvement initiatives, and required trainings
Qualifications:
- Active, unrestricted LPN license valid for practice
- At least 1 year of experience in home health, hospice, or community-based care
- Proficiency in Home Care Home Base (HCHB) software
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently managing a caseload in the field
Preferred:
- Experience with Medicare-certified home health agencies
- Thorough understanding of home health compliance and documentation standards
- Exceptional interpersonal skills with a patient-centered approach
Schedule & Workload:
- Monday through Friday, 8:00 AM 5:00 PM with occasional flexibility for patient needs
- Approximately 3 to 5 patient visits per day (2532 points weekly)
- Field-based work covering assigned territory; mileage reimbursement generally provided
