Responsibilities:
- Deliver skilled nursing care according to individualized care plans
- Monitor, document, and update patient progress and treatments as needed
- Administer medications and perform nursing treatments under state regulations
- Educate patients and their families on managing care and preventing disease
- Maintain accurate records using HomeCare HomeBase (HCHB) EMR
- Coordinate with RNs, therapists, and other providers to ensure holistic patient care
Qualifications:
- Active LPN license in good standing
- At least one year of recent home health nursing experience
- Proficient in HomeCare HomeBase (HCHB) electronic medical records
- Strong clinical assessment and documentation capabilities
- Reliable transportation for conducting home visits independently
- Ability to effectively manage a caseload and work autonomously
