Job Description

An experienced Licensed Practical Nurse (LPN) is needed to provide skilled nursing care to geriatric patients in a home health setting. This contract role offers an enriching opportunity to deliver compassionate care and support to patients in their homes within the Huntsville, AL area. The ideal candidate is proficient in HomeCare HomeBase (HCHB) EMR and has demonstrated expertise in managing and coordinating care for home health patients.

Responsibilities:

  • Deliver skilled nursing care according to individualized care plans in patients homes
  • Monitor, assess, and document patient progress, updating care plans as required
  • Administer medications and treatments compliant with state nursing guidelines
  • Educate patients and their families on disease management and preventive care
  • Maintain accurate and detailed records using HomeCare HomeBase (HCHB) EMR
  • Collaborate with RNs, therapists, and other healthcare professionals to ensure quality and coordinated care

Qualifications:

  • Active Licensed Practical Nurse (LPN) license
  • Minimum of one year recent and relevant home health experience
  • Proficiency in HomeCare HomeBase (HCHB) EMR system
  • Strong clinical assessment, critical thinking, and documentation abilities
  • Reliable transportation to conduct home visits
  • Ability to work independently while efficiently managing a patient caseload
