Qualifications:
- Active LPN license
- At least one year of recent experience in home health nursing
- Demonstrated proficiency with HomeCare HomeBase (HCHB) EMR
- Strong clinical assessment capabilities and meticulous documentation skills
- Reliable transportation to facilitate home visits in the community
- Ability to work autonomously while effectively managing a caseload
Responsibilities:
- Deliver professional skilled nursing care to patients in accordance with prescribed care plans
- Monitor, assess, and document patient progress, updating care plans when needed
- Administer medications and perform treatments in compliance with state regulations
- Educate patients and their families on disease prevention, care routines, and medication management
- Accurately maintain records and reports using HCHB EMR
- Communicate and collaborate with RNs, therapists, and healthcare colleagues to ensure holistic patient care
