Schedule/Hours:

Essential Qualifications:
- Current, active LPN license
- At least one year of recent home health experience
- Proficient in HomeCare HomeBase (HCHB) EMR
- Exceptional clinical assessment and documentation abilities
- Dependable transportation for home visits
- Ability to work independently and manage a patient caseload
Core Responsibilities:
- Deliver skilled nursing care in accordance with individualized patient care plans
- Monitor and document patient conditions, update care plans as needed
- Administer medications and perform treatments following state regulatory guidelines
- Educate patients and families about care management and disease prevention strategies
- Accurately maintain records using HomeCare HomeBase (HCHB) EMR
- Collaborate with registered nurses, therapists, and other healthcare team members to ensure integrated and effective patient care
