Essential Qualifications:

  • Current, active LPN license
  • At least one year of recent home health experience
  • Proficient in HomeCare HomeBase (HCHB) EMR
  • Exceptional clinical assessment and documentation abilities
  • Dependable transportation for home visits
  • Ability to work independently and manage a patient caseload

Core Responsibilities:

  • Deliver skilled nursing care in accordance with individualized patient care plans
  • Monitor and document patient conditions, update care plans as needed
  • Administer medications and perform treatments following state regulatory guidelines
  • Educate patients and families about care management and disease prevention strategies
  • Accurately maintain records using HomeCare HomeBase (HCHB) EMR
  • Collaborate with registered nurses, therapists, and other healthcare team members to ensure integrated and effective patient care
