If you are a caring and detail-oriented Licensed Practical Nurse with home health expertise, this opportunity is designed for you. In this field-based role, you will deliver skilled, compassionate care directly to patients within their homes, supporting their independence and recovery through individualized nursing interventions and patient education.

Schedule:

• Monday to Friday, 8:00 AM to 5:00 PM
• Occasional flexibility required for patient needs

Key Responsibilities:

• Conduct thorough nursing assessments and develop care plans tailored to each patients needs
• Administer medications, perform wound care, and manage IV therapies
• Regularly monitor patient conditions, reporting significant changes to the RN Case Manager or physician
• Educate patients and caregivers about disease management, medication adherence, and self-care
• Document all care activities and communication in Home Care Home Base (HCHB)
• Collaborate with the interdisciplinary team to adjust and coordinate plans of care
• Uphold infection prevention standards and safety protocols
• Participate in team meetings, case conferences, and quality initiatives

Qualifications:

• Active, unrestricted LPN license in the state
• Minimum of 1 year of experience in home health or similar care
• Proficiency with Home Care Home Base (HCHB)
• Valid drivers license, reliable transportation, auto insurance
• Current BLS certification
• Excellent time management and independent problem-solving skills

Preferred:

• Experience with Medicare-certified home health
• Strong compliance knowledge
• Effective communication and interpersonal abilities

$1,309 - $1,384/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
