An excellent opportunity is available for a Licensed Practical Nurse (LPN) with proven home health experience. This contract position is tailored for a compassionate clinician with a strong background in geriatric patient care, ready to make a meaningful impact in a home-based setting.
Schedule/Hours:
- Contract assignment
Key Qualifications:
- Current, valid LPN license
- At least one year of recent experience in a home health environment
- Hands-on proficiency with HomeCare HomeBase (HCHB) EMR
- Solid clinical assessment, documentation, and communication skills
- Reliable transportation to facilitate patient home visits
- Demonstrated ability to work independently and effectively manage a caseload
Primary Responsibilities:
- Deliver skilled nursing care to patients in their homes as per the established care plan
- Monitor patient conditions, update care plans, and maintain meticulous records in the HomeCare HomeBase (HCHB) platform
- Administer prescribed medications and perform state-approved treatments
- Provide education to patients and their families about disease prevention and care management
- Collaborate closely with RNs, therapists, and other healthcare professionals to ensure holistic patient care
