Job Description

A skilled and dedicated Licensed Practical Nurse (LPN) is needed to provide compassionate, high-quality home health care to patients within their residences. This contract position places you in a pivotal role, supporting patients recovery and independence while collaborating with an interdisciplinary healthcare team. Prior experience with Home Care Home Base (HCHB) and a keen understanding of home health compliance will set you up for success in this dynamic environment.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

  • Deliver skilled nursing care in patients' homes, assessing, planning, and implementing individualized care plans.
  • Administer medications, conduct wound care, and perform IV therapy or other clinical treatments as ordered.
  • Monitor patients conditions, promptly communicating changes to the RN Case Manager or physician.
  • Educate patients and families on health management, medication, and self-care.
  • Ensure thorough, accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with the interdisciplinary team for comprehensive care coordination.
  • Uphold infection control measures and prioritize patient safety at all times.
  • Participate in case conferences, training, and quality improvement initiatives.

Required Qualifications:

  • Active, unrestricted LPN license in the state of practice.
  • At least 1 year of experience in home health, hospice, or a similar setting.
  • Proficiency with Home Care Home Base (HCHB) electronic charting.
  • Valid drivers license, reliable transportation, and current insurance.
  • BLS certification (Basic Life Support).
  • Excellent clinical, time management, and communication skills.
  • Comfort working independently in a field-based role and managing a dynamic caseload.

Preferred Skills:

  • Experience with Medicare-certified home health agencies.
  • Strong knowledge of compliance and home health documentation standards.
  • Patient-focused with outstanding interpersonal skills.
