Job Description

Provide compassionate, skilled nursing care as a Home Health Licensed Practical Nurse (LPN) serving patients in their residences within the Hephzibah, GA area. This full-time contract role offers the opportunity to promote patient independence and recovery through personalized care while maintaining close communication with patients, families, and the interdisciplinary healthcare team.

Working Monday through Friday from 8:00 AM to 5:00 PM, you will manage a caseload equivalent to 35 patient visits daily, utilizing the Home Care Home Base (HCHB) system for thorough documentation. Reliable transportation is essential, with mileage reimbursement typically provided.

Key Responsibilities:

  • Perform skilled nursing assessments and develop individualized care plans for home health patients
  • Administer medications, provide wound care, and manage IV therapy or other clinical procedures as prescribed
  • Monitor patient conditions and promptly report any significant changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medications, and self-care techniques
  • Accurately document all patient interactions and care activities using HCHB
  • Collaborate with healthcare team members to ensure adherence to care plans and compliance standards
  • Uphold infection control protocols and patient safety throughout visits
  • Engage in case conferences, quality improvement efforts, and mandatory trainings

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum of 1 year experience in home health, hospice, or community-based care
  • Proficiency with Home Care Home Base (HCHB) charting system
  • Valid drivers license, reliable insured vehicle
  • Current Basic Life Support (BLS) certification
  • Excellent clinical judgment, communication, and time-management skills
  • Ability to independently manage a field-based caseload

Preferred:

  • Experience with Medicare-certified home health agencies
  • Strong knowledge of home health compliance and documentation standards
  • Patient-centered approach and superior interpersonal skills
