Seeking an experienced and compassionate Licensed Practical Nurse for a dynamic home health opportunity. In this field-based role, you will deliver high-quality, patient-centered nursing services to individuals in the comfort of their homes. This position is ideal for motivated LPNs driven by patient advocacy, education, and helping others regain their independence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)
Key Qualifications:
- Active, unrestricted LPN license for the state
- At least 1 year of experience in home health, hospice, or a similar community care setting
- Proficiency using Home Care Home Base (HCHB) charting system (required)
- Valid drivers license, reliable transportation, and current auto insurance
- BLS (Basic Life Support) certification
- Demonstrated clinical judgment, time management, and strong communication skills
- Ability to thrive independently and manage a dynamic caseload
Preferred Experience:
- Familiarity with Medicare-certified home health agency standards and documentation requirements
- Solid grasp of home health compliance
- Excellent interpersonal skills and a dedication to patient-centered care
Primary Duties:
- Conduct skilled nursing visits, develop and implement individualized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as needed
- Monitor patients conditions, promptly report concerns, and update care plans as directed
- Educate patients and caregivers on disease management, medication, and self-care
- Accurately document all care and communication using HCHB
- Collaborate with the healthcare team to ensure coordinated, compliant care delivery
- Actively participate in case conferences, quality improvement, and ongoing training
