Advance your nursing career by providing high-quality, compassionate care to patients in their homes as a Home Health LPN. In this contract, field-based position, you will support patient independence and recovery by delivering skilled nursing services and collaborating closely with patients, families, and the interdisciplinary healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Responsibilities:

• Conduct comprehensive nursing visits, assess patient needs, and develop individualized care plans
• Administer medications, wound care, IV therapy, and clinical interventions as ordered
• Monitor patient condition changes and report findings to the RN Case Manager or physician
• Provide education to patients and caregivers on disease management and self-care
• Document all assessments and care in Home Care Home Base (HCHB)
• Coordinate with interdisciplinary team members to ensure optimal care and plan compliance
• Maintain strict infection control and safety standards at all times
• Participate in case conferences, training, and quality improvement initiatives

Requirements:

• Active, unrestricted LPN license in the state
• Minimum 1 year of experience in home health, hospice, or community-based care
• Proficient in Home Care Home Base (HCHB) documentation platform
• Valid drivers license and reliable vehicle with active insurance
• Current BLS certification
• Able to work independently in a field setting, managing a dynamic caseload of approximately 35 patient visits per day
• Strong clinical, organizational, and communication skills

Preferred:

• Experience with Medicare-certified home health agencies
• Knowledge of home health compliance and documentation standards
• Patient-centered, with excellent interpersonal skills
