Job Description
Deliver compassionate care and make a meaningful impact as a Licensed Practical Nurse specializing in home health services for geriatric patients. This contract opportunity is tailored for LPNs experienced in delivering top-quality care in patients homes and eager to continue growing their clinical skills. Proficiency with HomeCare HomeBase (HCHB) EMR is required.
Schedule/Hours:
- Full-time, contract assignment
Key Qualifications:
- Active and unrestricted LPN license
- At least one year of recent experience in home health nursing
- Demonstrated proficiency using HomeCare HomeBase (HCHB) EMR
- Excellent clinical assessment and documentation abilities
- Reliable transportation for in-home patient visits
- Strong organizational skills and the ability to manage a caseload independently
Primary Responsibilities:
- Provide skilled and compassionate nursing care according to individualized care plans
- Monitor patient conditions, update care plans, and document all interventions thoroughly in HCHB
- Administer medications and execute treatments in accordance with state guidelines
- Educate patients and their families on ongoing care management and strategies for disease prevention
- Collaborate closely with registered nurses, therapists, and other clinicians to ensure holistic, coordinated care
