Job Description

Deliver essential care to geriatric patients as a Licensed Practical Nurse specializing in home health. This contract role is suited for a dedicated professional who thrives in independent settings and is committed to high-quality, compassionate care for patients in their homes.

Schedule/Hours:

  • Contract role with potential for extension

Qualifications:

  • Valid and active LPN license
  • At least one year of recent experience providing care in a home health setting
  • Demonstrated proficiency in HomeCare HomeBase (HCHB) EMR
  • Exceptional clinical assessment and documentation capabilities
  • Strong organizational and time-management skills to handle independent caseloads efficiently
  • Reliable transportation to travel to patient homes

Responsibilities:

  • Deliver skilled nursing care to patients in accordance with personalized care plans
  • Monitor, assess, and document patient conditions, updating care plans as health needs evolve
  • Administer medications and treatments safely within state regulations
  • Educate patients and their families on disease prevention and effective health management
  • Maintain accurate, up-to-date electronic records using HomeCare HomeBase (HCHB) EMR
  • Work collaboratively with RNs, therapists, and other healthcare professionals to support comprehensive patient care
