Job Description

Make a difference in patients lives by providing skilled care in a home health setting as a Licensed Practical Nurse (LPN). This full-time contract opportunity focuses on patient-centered nursing for individuals in their homes, supporting recovery, independence, and education.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility for patient needs

Key Qualifications:

  • Active, unrestricted LPN license in applicable state
  • At least one year of experience in home health, hospice, or other community-based care
  • Proficient with Home Care Home Base (HCHB) for electronic documentation
  • Valid drivers license, reliable transportation, and current insurance
  • Current BLS certification
  • Strong clinical, organizational, and communication skills
  • Ability to manage a dynamic caseload, typically 2532 points per week (35 visits per day)

Preferred Experience:

  • Prior work in Medicare-certified agencies
  • Comprehensive knowledge of home health regulations, compliance, and documentation standards
  • Excellent interpersonal skills with a commitment to patient education

Primary Responsibilities:

  • Deliver skilled nursing visits, assess patient needs, and implement individualized care plans
  • Administer medications, wound care, and clinical interventions per physician orders
  • Observe and communicate changes in patient conditions to the RN Case Manager or physician
  • Educate patients and caregivers in disease process, medication, and self-care strategies
  • Chart all care and communication accurately with HCHB
  • Maintain infection control practices and ensure patient safety in the home environment
  • Coordinate care with the interdisciplinary team and participate in ongoing quality improvement
