Job Description

Deliver skilled nursing care as a Licensed Practical Nurse (LPN) within patients homes, focusing on promoting independence, education, and recovery. This full-time contract role requires managing a caseload of home health patients, using Home Care Home Base (HCHB) for documentation, and working collaboratively with an interdisciplinary team to ensure high-quality, compliant care.

Location:
Positioned in Harlem, GA, this role offers a field-based work environment serving patients in the surrounding community. Harlem is known for its welcoming atmosphere, with convenient access to urban amenities while maintaining a small-town charm. Reliable transportation is essential for fulfilling patient visits within the assigned territory.

Responsibilities:

  • Conduct thorough skilled nursing visits to assess, plan, and implement individualized care plans.
  • Administer medications, perform wound care, and manage IV therapy or other clinical treatments as prescribed.
  • Monitor patients for changes and communicate findings to RN Case Managers or physicians promptly.
  • Educate patients and caregivers on treatment regimens, medication management, and self-care.
  • Accurately document all clinical care and patient interactions in Home Care Home Base (HCHB).
  • Collaborate with healthcare team members to coordinate care and ensure adherence to compliance standards.
  • Uphold infection control practices and prioritize patient safety during home visits.
  • Participate in quality improvement initiatives, case conferences, and required training sessions.

Qualifications & Experience:

  • Active, unrestricted LPN license in the practicing state.
  • Minimum one year of experience in home health, hospice, or community care settings.
  • Proficiency with Home Care Home Base (HCHB) charting system.
  • Valid drivers license, reliable insured vehicle for travel to patient homes.
  • Current BLS (Basic Life Support) certification.
  • Strong clinical judgment, communication, and time management skills.
  • Ability to work independently managing a dynamic caseload in the field.

Preferred:

  • Background with Medicare-certified home health agencies.
  • Thorough knowledge of home health compliance and documentation requirements.
  • Exceptional interpersonal skills with a compassionate, patient-centered care approach.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs.
  • Caseload equivalent to roughly 35 patient visits daily (2532 points weekly).
