Job Description
Provide compassionate, skilled nursing care as a Home Health Licensed Practical Nurse (LPN) serving patients in their residences. This full-time, contract role requires expertise in delivering patient-centered care designed to promote recovery, independence, and education while maintaining clear communication with patients, families, and the healthcare team.
Your responsibilities will include conducting skilled nursing visits to assess and implement individualized care plans, administering medications, wound care, and managing clinical interventions such as IV therapy. You'll monitor patient conditions closely, report changes to the RN Case Manager or physician promptly, and educate patients and caregivers about disease processes and self-care techniques. Accurate documentation using Home Care Home Base (HCHB) is essential, as is collaboration with interdisciplinary teams to ensure compliance and quality care. Participation in case conferences, training, and quality improvement activities rounds out the role.
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or similar community settings
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, effective time management, and communication skills
- Ability to work independently managing a dynamic caseload in a field-based environment
Preferred:
- Previous experience with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Exceptional interpersonal skills with a patient-focused approach
Schedule and Workload:
- Monday to Friday, 8:00 AM 5:00 PM (flexibility occasionally required for patient needs)
- Approximately 35 patient visits daily (2532 points per week)
- Field-based, traveling within an assigned territory; mileage reimbursement typically provided
