Job Description
Make a meaningful difference delivering high-quality in-home care as a Licensed Practical Nurse specializing in home health services. This contract position offers the opportunity to support and empower patients to achieve greater independence in the comfort of their own homes. Youll coordinate care plans, perform essential nursing interventions, and build strong, trust-based relationships with patients and their families.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to accommodate patient needs
Key Responsibilities:
- Provide skilled nursing visits, creating and implementing individualized care plans
- Administer medications, wound care, IV therapies, and other clinical interventions
- Monitor and document changes in patient conditions; promptly communicate with the RN Case Manager or physician
- Educate patients and caregivers on disease processes and self-care management techniques
- Accurately complete all charting in Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary healthcare team to coordinate holistic care
- Uphold rigorous infection control standards and ensure ongoing patient safety
- Participate in case conferences, quality initiatives, and required training
Required Qualifications:
- Active, unrestricted LPN license
- Minimum 1 year of hands-on experience in home health or similar setting
- Proficiency using Home Care Home Base (HCHB)
- Valid drivers license, reliable vehicle, and current insurance
- Current BLS certification
- Excellent judgment, time management, and communication skills
- Comfort working independently and managing dynamic caseloads
Preferred Background:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation best practices
- Patient-centered, collaborative approach
