Enjoy the opportunity to make a real impact in patients lives by delivering home-based nursing care as a Home Health LPN. In this field-based contract position, youll work autonomously throughout your assigned territory, caring for patients in the comfort of their homes while fostering patient independence and positive outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM, with occasional flexibility based on patient needs
Key Responsibilities:
- Conduct routine skilled nursing visits, assessing and managing individualized patient care plans
- Administer medications, provide wound care, and manage IV therapies or other clinical interventions per provider orders
- Monitor and document changes in patients conditions, communicating promptly with the RN Case Manager or physician
- Educate patients and caregivers on diagnoses, medication regimens, and self-care to promote optimal health and recovery
- Complete accurate, timely documentation using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary care team to maintain compliance, coordinate services, and support patient goals
- Participate actively in case conferences, ongoing training, and quality improvement initiatives
Qualifications:
- Active, unrestricted LPN license in Georgia
- At least 1 year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) charting system
- Valid drivers license and reliable insured vehicle
- Current BLS certification
- Strong assessment, time management, and communication skills
- Ability to work independently and adjust to patient and caseload needs
Preferred:
- Experience with Medicare-certified home health agencies
- Knowledge of home health regulatory and documentation standards
