Providing compassionate, high-quality nursing care in patients' homes, this opportunity is designed for experienced Licensed Practical Nurses (LPN) who specialize in home health. The role involves managing a caseload of patients within a defined territory, ensuring compliance with regulatory standards, and collaborating closely with the interdisciplinary healthcare team.

Key Responsibilities:
- Conduct skilled nursing visits to assess patient conditions and develop individualized care plans
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy
- Monitor patient status and communicate changes promptly with physicians and RN Case Managers
- Educate patients and caregivers on disease processes, medication management, and self-care techniques
- Document nursing assessments and care activities accurately using Home Care Home Base (HCHB)
- Collaborate with the healthcare team to ensure coordinated, compliant care delivery
- Maintain infection control and patient safety standards at all times
- Participate in case conferences and quality improvement initiatives
Qualifications and Desired Experience:
- Active, unrestricted LPN license in the state of Georgia
- Minimum of 1 year experience in home health, hospice, or a related community care setting
- Proficiency in the Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable insured vehicle, and dependable transportation
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based environment
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Deep understanding of home health compliance and documentation standards
- Patient-centered approach with excellent interpersonal skills
Contract Details:
- Full-time contract position
- Monday through Friday, 8:00 AM to 5:00 PM, with occasional flexibility based on patient needs
- Approximately 35 patient visits per day, totaling 2532 points per week
- Mileage reimbursement typically provided
