Job Description
Provide compassionate, high-quality nursing care as a Licensed Practical Nurse (LPN) in the home health setting. This field-based role involves managing a caseload of patients within a defined territory, delivering skilled nursing assessments, treatments, and education directly in patients homes. You will work collaboratively with the interdisciplinary healthcare team while upholding compliance with home health regulations and documentation standards.
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care for home health patients
- Administer medications, perform wound care, and manage IV therapy or clinical interventions as prescribed
- Monitor and report changes in patient conditions to RN Case Manager or physician
- Educate patients and caregivers on disease management, medication adherence, and self-care practices
- Document assessments, care delivery, and communication accurately using Home Care Home Base (HCHB)
- Coordinate care with interdisciplinary team members and adhere to infection control and safety protocols
- Participate in case conferences, quality improvement initiatives, and mandatory trainings
Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum one year of experience in home health, hospice, or community-based care
- Proficiency with Home Care Home Base (HCHB) electronic charting system
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based environment managing a variable caseload
Preferred Experience:
- Prior work with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation standards
- Excellent interpersonal skills focused on patient-centered care
Schedule: Monday Friday, 8:00 AM 5:00 PM, with occasional flexibility to meet patient needs
Workload: Approximately 35 patient visits daily, totaling 2532 points per week
