Job Description

Deliver essential nursing care in a home health environment, supporting geriatric patients and their families with compassion and clinical expertise. An experienced LPN with home health expertise and proficiency in HomeCare HomeBase (HCHB) EMR is sought for this assignment.

Schedule/Hours:

  • Contract position; full-time hours
  • Opportunity for contract extension

Key Qualifications:

  • Active LPN license
  • At least one year of recent home health experience
  • Proficient in HomeCare HomeBase (HCHB) EMR
  • Strong assessment, clinical, and documentation abilities
  • Reliable transportation for travel to patient homes
  • Self-motivated and comfortable managing a caseload independently

Primary Responsibilities:

  • Deliver skilled nursing care as outlined in patient care plans
  • Monitor, assess, and document patient progress, updating care plans as necessary
  • Administer medications and perform treatments in compliance with state regulations
  • Educate patients and family members on disease prevention and home care management
  • Accurately maintain records and documentation in HCHB EMR system
  • Coordinate care and communicate effectively with interdisciplinary healthcare teams
