An exciting opportunity is available for a dedicated Home Health Licensed Practical Nurse (LPN) to make a tangible impact by delivering skilled nursing care to patients in their homes. This role requires professionalism, adaptability, compassion, and a commitment to helping individuals maintain independence while recovering from illness or surgery. Youll work collaboratively within an interdisciplinary healthcare team to ensure optimal outcomes for every patient on your caseload.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility based on patient needs
Qualifications:
- Active, unrestricted LPN license in the applicable state
- Minimum of 1 year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) charting system
- Current BLS certification
- Valid drivers license, reliable transportation, and current vehicle insurance
- Ability to work independently, demonstrating strong clinical judgment, communication, and organization skills
- Preferred: Experience with Medicare-certified agencies and a solid understanding of home health compliance/documentation
Key Responsibilities:
- Conduct in-home skilled nursing assessments and provide direct care based on individualized plans
- Administer medications, perform wound care, and manage complex care interventions as prescribed
- Monitor, document, and report changes in patients health conditions to the RN Case Manager or physician
- Educate patients and caregivers on disease processes, medication regimens, and strategies that promote self-care
- Maintain thorough, compliant electronic documentation using HCHB
- Uphold infection control and patient safety standards
- Actively participate in interdisciplinary case conferences and quality improvement initiatives
