This contract position focuses on supporting geriatric patients, ensuring high-quality, individualized care in the comfort of their residences. The ideal candidate has expertise in home health and a solid grasp of the HomeCare HomeBase (HCHB) EMR platform.
Key Qualifications:
- Active LPN license in good standing
- At least one year of recent experience in a home health LPN role
- Proficient with HomeCare HomeBase (HCHB) EMR
- Strong abilities in clinical assessment and documentation
- Dependable transportation for conducting home visits
- Demonstrated independence and ability to efficiently manage a caseload
Core Responsibilities:
- Provide skilled nursing interventions as prescribed in individualized care plans
- Document patient assessments, interventions, and outcomes using HCHB EMR
- Administer medications and perform treatments in compliance with state regulations and protocols
- Educate patients and their families on clinical care, home safety, and disease prevention strategies
- Routinely assess patient progress and communicate changes to the care team
- Collaborate closely with RNs, therapists, and other healthcare providers for holistic patient support
