Providing skilled nursing care in patients homes requires compassion, clinical expertise, and autonomy. This full-time contract role based in Grandview, MO offers an opportunity to apply your Licensed Practical Nurse (LPN) skills within a rewarding home health setting.
As a Home Health LPN, you will manage a caseload of patients, conducting skilled nursing visits that include medication administration, wound care, and clinical interventions. You will educate patients and caregivers on disease management while collaborating closely with an interdisciplinary team to coordinate care. Accurate documentation using Home Care Home Base (HCHB) is critical to maintaining compliance and ensuring excellent patient outcomes.
Key Responsibilities:
- Provide skilled nursing assessments and individualized care plans in patients homes
- Administer medications, perform wound care, and manage IV therapy as ordered
- Monitor patient conditions, promptly reporting any changes to RNs or physicians
- Educate patients and caregivers on medication management and self-care techniques
- Accurately document all care and communications using HCHB
- Collaborate with the healthcare team to ensure coordinated and compliant care
- Uphold infection control standards and patient safety protocols
Qualifications & Experience:
- Active, unrestricted LPN license in the state of practice
- Minimum 1 year of home health, hospice, or community-based care experience
- Proficiency with Home Care Home Base (HCHB) software
- Valid drivers license, reliable vehicle, and current insurance
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in the field managing a dynamic patient caseload
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Thorough understanding of home health compliance and documentation standards
- Excellent interpersonal skills and a patient-focused approach
Work Schedule:
- Monday to Friday, 8:00 AM 5:00 PM, with occasional schedule flexibility to meet patient needs
- Approximately 2532 points weekly, equating to 35 patient visits daily
