Job Description
Step into a rewarding contract opportunity supporting patients as a Home Health Licensed Practical Nurse (LPN) specializing in home-based care. In this dynamic field role, youll provide hands-on nursing care, promote patient independence, and ensure optimal recovery outcomes. Your expertise in Home Health and proficiency with the Home Care Home Base (HCHB) platform will be crucial to delivering high-standard care and documentation.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility based on patient needs
Qualifications:
- Active and unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or community-based care
- Proficient in Home Care Home Base (HCHB) electronic charting
- Valid drivers license and reliable transport with current insurance
- Current BLS (Basic Life Support) certification
- Excellent clinical judgment, communication, and time management skills
- Ability to work independently and efficiently manage a caseload
Preferred Experience:
- Expertise with Medicare-certified home health agencies
- In-depth knowledge of home health compliance and documentation practices
- Patient-focused approach and strong relationship-building skills
Responsibilities:
- Conduct skilled nursing visits, develop and implement individualized care plans
- Administer medications, perform wound care, manage IV therapy, and other clinical tasks
- Assess patient status and promptly communicate changes to the RN Case Manager or physician
- Educate patients and caregivers about care procedures and self-management
- Complete thorough documentation in Home Care Home Base (HCHB)
- Collaborate with interdisciplinary team members for coordinated patient care
- Uphold infection control and patient safety standards
- Participate in case conferences, quality improvement initiatives, and required trainings
