Job Description

Delivering patient-centered care in a home health setting, this Licensed Practical Nurse role offers the chance to make a meaningful impact in the lives of individuals managing recovery and chronic health concerns in their own homes. As a field-based position, you will travel within your assigned area, providing skilled nursing services and educating patients and families to foster independence and better outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required to meet patient needs

Key Responsibilities:

  • Conduct skilled nursing visits, developing and implementing individualized care plans for home health patients
  • Administer medications, monitor clinical interventions such as IV therapy, and perform wound care
  • Evaluate and document changes in patient condition, reporting to the RN Case Manager or physician as needed
  • Provide education to patients and caregivers on disease processes, medications, and self-care practices
  • Utilize Home Care Home Base (HCHB) for accurate, timely electronic documentation
  • Coordinate care with the interdisciplinary healthcare team to ensure regulatory compliance and patient safety
  • Maintain infection control standards and uphold the highest quality of care in every interaction
  • Participate in case conferences, ongoing education, and quality improvement initiatives

Qualifications:

  • Active, unrestricted LPN license (state of practice)
  • At least one year of experience in home health, hospice, or related fieldwork
  • Proficiency in Home Care Home Base (HCHB) electronic documentation system
  • Valid drivers license, reliable insured vehicle, and willingness to travel within an assigned territory
  • Current BLS certification
  • Strong clinical assessment, time management, and communication abilities
  • Ability to work independently and effectively manage a field-based caseload
  • Preferred: Experience with Medicare-certified agencies and strong understanding of home health compliance
