Job Description

Deliver compassionate care and empower patient independence as a Licensed Practical Nurse supporting clients in their homes. In this contract position, youll provide field-based nursing services, conducting regular visits and supporting individuals on their journey to recovery within the comfort of their surroundings.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM (occasional flexibility for patient needs)

Key Responsibilities:

  • Complete skilled nursing assessments and implement individualized care plans for home health patients.
  • Administer prescribed medications, manage IV therapy, and perform wound care as required.
  • Monitor and document any changes in patient condition, promptly reporting to RN Case Managers or physicians.
  • Educate patients and caregivers on disease management, medication routines, and self-care strategies.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Coordinate with the interdisciplinary healthcare team to ensure seamless patient care and compliance.
  • Uphold infection control procedures and maintain patient safety on every visit.
  • Participate in ongoing staff training, case conferences, and quality improvement activities.

Required Qualifications:

  • Active, unrestricted LPN license for the state of assignment.
  • At least 1 year of experience in home health, hospice, or a related community-based care setting.
  • Proficiency in charting and workflow using Home Care Home Base (HCHB).
  • Valid drivers license, reliable transportation, and current auto insurance.
  • Current Basic Life Support (BLS) certification.
  • Ability to work independently and manage a dynamic caseload.
  • Strong clinical judgment and effective communication skills.

Preferred:

  • Experience with Medicare-certified home health agencies.
  • Strong knowledge of home health documentation and compliance.
