Job Description
Provide exceptional nursing care to patients in their homes as a Licensed Practical Nurse specializing in home health. This role demands compassion, clinical expertise, and strong communication to support patients independence and recovery within a community setting.
Key Responsibilities:
- Conduct skilled nursing visits to assess and implement individualized care plans.
- Administer medications, wound care, IV therapy, and other clinical interventions as prescribed.
- Monitor patient conditions, promptly reporting changes to the RN Case Manager or physician.
- Educate patients and caregivers on disease management, medication usage, and self-care strategies.
- Accurately document all care and communication using Home Care Home Base (HCHB).
- Collaborate with interdisciplinary teams to ensure coordinated, compliant patient care.
- Uphold infection control and safety standards during all visits.
- Participate in case conferences, quality improvement initiatives, and required trainings.
Qualifications and Experience:
- Active, unrestricted LPN license for the state of practice.
- Minimum one year of experience in home health, hospice, or community-based nursing.
- Proficiency with Home Care Home Base (HCHB) charting system.
- Valid drivers license, reliable insured vehicle, and dependable transportation.
- Current Basic Life Support (BLS) certification.
- Strong clinical judgment, time management, and communication skills.
- Ability to independently manage a dynamic caseload in a field-based environment.
Preferred:
- Experience with Medicare-certified home health agencies.
- Deep understanding of home health compliance and documentation standards.
- Outstanding interpersonal skills with a patient-focused approach.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM, with occasional flexibility based on patient needs.
- Approximately 35 patient visits per day (2532 points weekly).
