Make a difference in the lives of patients by providing professional, patient-centered care in a home setting. This full-time contract opportunity focuses on supporting individuals in their recovery and maintaining independence. As a Home Health Licensed Practical Nurse, youll travel to patients homes, work closely with an interdisciplinary team, and use your skills to deliver hands-on care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional schedule flexibility based on patient needs
Responsibilities:
- Conduct skilled nursing visits and develop individualized care plans
- Administer medications, perform wound care, and manage IV therapy according to orders
- Monitor and promptly report changes in patients conditions
- Educate patients and caregivers on disease processes, medications, and self-care techniques
- Accurately document visits and interventions in Home Care Home Base (HCHB)
- Collaborate with clinicians to coordinate patient-centered care
- Adhere to infection control and patient safety protocols
- Participate in case conferences and ongoing training
Qualifications:
- Active and unrestricted LPN license in the state of practice
- Minimum of 1 year of home health, hospice, or related experience
- Proficiency in Home Care Home Base (HCHB) for documentation
- Valid drivers license; reliable vehicle with current insurance
- Current BLS certification
- Strong clinical judgment, time management, and the ability to work independently
- Preferred: Experience with Medicare-certified home health agencies and knowledge of compliance standards
