Job Description
Provide skilled nursing care as a Home Health Licensed Practical Nurse (LPN) to patients in their private residences, focusing on promoting independence, recovery, and education. This full-time contract position involves managing a caseload of 2532 weekly points, approximately 35 patient visits per day, with a Monday through Friday schedule from 8:00 AM to 5:00 PM and occasional flexibility for patient needs.
Key Responsibilities:
- Conduct in-home patient assessments and implement individualized care plans
- Administer medications, wound care, IV therapy, and other clinical treatments as ordered
- Monitor patient conditions and communicate changes promptly to the RN Case Manager or physician
- Educate patients and caregivers on disease management and self-care techniques
- Accurately document patient care using the Home Care Home Base (HCHB) system
- Collaborate with interdisciplinary teams to ensure coordinated, compliant care
- Maintain infection control standards and patient safety throughout care visits
- Participate in case conferences and quality improvement activities
Qualifications:
- Current, unrestricted LPN license in the state of practice
- Minimum of 1 year experience in home health, hospice, or community-based nursing
- Proficiency in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable insured vehicle, and ability to travel within assigned territory
- Current Basic Life Support (BLS) certification
- Strong clinical judgment, communication, and time management skills
- Ability to work independently in a field-based setting
Preferred Qualifications:
- Experience with Medicare-certified home health agencies
- Thorough understanding of home health compliance and documentation
- Exceptional interpersonal skills and patient-focused mindset
