Job Description

Provide skilled nursing care as a Home Health Licensed Practical Nurse (LPN) to patients in their private residences, focusing on promoting independence, recovery, and education. This full-time contract position involves managing a caseload of 2532 weekly points, approximately 35 patient visits per day, with a Monday through Friday schedule from 8:00 AM to 5:00 PM and occasional flexibility for patient needs.

Key Responsibilities:

• Conduct in-home patient assessments and implement individualized care plans
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor patient conditions and communicate changes promptly to the RN Case Manager or physician
• Educate patients and caregivers on disease management and self-care techniques
• Accurately document patient care using the Home Care Home Base (HCHB) system
• Collaborate with interdisciplinary teams to ensure coordinated, compliant care
• Maintain infection control standards and patient safety throughout care visits
• Participate in case conferences and quality improvement activities

Qualifications:

• Current, unrestricted LPN license in the state of practice
• Minimum of 1 year experience in home health, hospice, or community-based nursing
• Proficiency in Home Care Home Base (HCHB) charting system
• Valid drivers license, reliable insured vehicle, and ability to travel within assigned territory
• Current Basic Life Support (BLS) certification
• Strong clinical judgment, communication, and time management skills
• Ability to work independently in a field-based setting

Preferred Qualifications:

• Experience with Medicare-certified home health agencies
• Thorough understanding of home health compliance and documentation
• Exceptional interpersonal skills and patient-focused mindset
